Optical Diagnostics collects personal information when an individual interacts with Optical Diagnostics, such as sending an e-mail, filling out an on-line form, and purchasing a product.  The type of personal information collected will vary but may include name, address, phone number, fax number, birth date, billing and delivery information, e-mail address, computer ID numbers, health information, profession, and other demographic information.

With Whom We May Share Information

If you give your computer ID number, it is only used to copy-protect Optical Diagnostics' software.  It will not be shared with a third party without your consent.

 

Your health case may be discussed with a third party, used for scientific research purposes (including presenting in a scientific journal or conference), or used for marketing purposes (e.g., on the web-site).  However, the data will be presented anonymously (i.e., no name, address, phone/fax number, birth date (although age might be provided), billing and delivery information, e-mail address, and computer ID number).

 

Optical Diagnostics may disclose your personal information if required to do so by law or in the good-faith belief that such action is necessary to: (a) conform to legal requirements; (b) protect and defend the rights or property of Optical Diagnostics; or, (c) protect the personal safety of Optical Diagnostics personnel or members of the public in urgent circumstances.

Optical Diagnostics uses common anti-virus and anti-spyware software to protect against unauthorized electronic access to your personal information.  Note that messages sent via the Internet (e-mails, web-forms) is not completely secure and can be intercepted by a third party. If you are concerned about sending your personal information to us via the Internet, you can use another method such as regular mail.

We treat your personal information in a confidential manner. Our employees and other authorized personnel are required to protect the confidentiality of your personal information. We restrict access to personal information about you to only those individuals who have a business reason to know the information, such as in order to provide services for you.
